A night guard, clinically termed an occlusal splint, is a custom-fitted appliance placed between the upper and lower teeth in patients with signs of bruxism (involuntary clenching or grinding during sleep). It is typically fabricated for the upper arch and produced from individual impressions.

Function of the Splint

The primary function of the splint is to distribute load evenly across the dental arch when masticatory muscles contract, reducing abrasive contact on enamel surfaces. It also lowers compressive force on the temporomandibular joint and may decrease the intensity of morning muscle tension.

A splint is not a treatment that eliminates bruxism itself. It serves a protective role for existing tooth structure and the TMJ. When underlying factors such as stress, sleep patterns or occlusal relationships require attention, a broader plan is developed under bruxism treatment.

Clinical Indications

In patients presenting with nocturnal grinding, the following findings are commonly evaluated during the decision for splint therapy:

  • Marked fatigue in jaw or facial muscles upon waking
  • Visible occlusal wear or chipping
  • TMJ sounds, pain or restricted opening
  • Recurrent temporal headache
  • Premature wear of existing restorations

When TMJ involvement is significant, assessment is combined with TMJ treatment.

Fabrication Process

After clinical examination, impressions are taken from both arches. The laboratory produces a custom splint, which is delivered at a subsequent visit. Minor adjustments to occlusal contact points may be performed chairside. Adaptation varies between patients, with most reporting comfortable use within the first nights.

Daily Use

Consistent use is important for the splint to fulfill its function. It is placed before sleep and removed in the morning, then cleaned with a brush and stored in a dry, ventilated container. Periodic recall appointments allow assessment of fit and wear.

When a Splint Alone Is Insufficient

In some cases, splint use alone does not adequately control bruxism findings. Muscle relaxation exercises, physiotherapy referral, stress management or occlusal adjustments may be incorporated. This stepwise approach is explained in detail on the bruxism treatment page.
